I would expect to see the same droids (I think they're having them play roles in all of them just like Threepio and Artoo in the saga proper) and entirely different cast of party members, with your main character somehow connected to Revan and/or the Exile.

Perhaps that could be one possibility out of several. KotOR II had a group of questions regarding the identity of Revan and outcome of KotOR I early in the game... so the whole game would be more open-ended dependant on how one answered the early game questions about Revan and the Exile. That would be how I expect it would go...
Revan and Bastila's son perhaps would be one possibility... dependant on your choice. Could be Revan and Carth's son if you chose that Revan was female... If you choose a good Exile, then perhaps a more heretical acolyte of the Exile would be your "Darth Traya" styled figure.
I'd rather they kept the choices in however... even if it had little real effect over the whole game. (All that was effected by KotOR I really was just what particular cameos from KotOR I appeared)

Seriously though, I'd like to see Coruscant, Mustafar, maybe a return to Taris and a follow-up with the Undercity people and the Promised Land quest... Naboo might be cool, if done right. Hoth would be killer, or perhaps that other ice planet seen in Clone Wars where the crystal caves were...
